5 things to check before ordering transfer chairs for your facility

A procurement engineer's checklist: caster lock type, lift mechanism, weight capacity and frame, seat design, and what you actually get in the box. Skip the brochure claims, ask these five questions instead.

A transfer chair is a daily-use device. Your staff will push it, lock it, unlock it, and reposition it dozens of times per shift. Residents will sit in it for meals, roll into showers with it, and depend on it during the most vulnerable moments of their day — bed-to-chair and chair-to-toilet transfers. When procurement treats transfer chairs as a commodity line item, the cost shows up later: in caregiver injuries, equipment downtime, and resident falls.

The numbers back this up. According to OSHA's guidelines on safe patient handling[1], healthcare workers experience musculoskeletal injuries at rates exceeding construction and manufacturing — and manual patient transfers are the leading cause. A properly spec'd transfer device replaces the manual lift, directly reducing the facility's largest source of workplace injury.

Five checks before you place your order

  • Does the caster lock prevent both wheel rotation and swivel — or just one?
  • Is the lift hydraulic or electric, and what happens when the power goes out?
  • What is the actual weight capacity, and what is the frame made of?
  • Does the seat design work for toileting, showering, and dining — not just transport?
  • What comes in the box, and what accessories cost extra?

1. Caster lock: the safety component nobody checks

During a pivot transfer — bed to chair, chair to toilet — the chair must be completely stationary. A caster that locks the wheel from rolling but still allows the swivel to rotate is not locked. The chair can spin under the resident's shifting weight, and a spinning chair during a transfer is a fall waiting to happen.

The question to ask your supplier is specific: "Does the caster lock prevent both wheel rotation and swivel movement?" If the answer is "it locks the wheels" without mentioning swivel, assume single-lock. A dual-lock caster immobilizes the chair in both axes. This is the difference between a transfer device and a chair on wheels.

Also check: how many casters lock? Two locking casters is the minimum for a transfer chair. Four is better — it eliminates any pivot point. And test the lock engagement yourself: a pedal or lever that requires the caregiver to bend down and reach under the chair is a productivity drain. Look for toe-tap engagement that a caregiver can operate without bending.

2. Lift mechanism: hydraulic vs. electric

Transfer chairs come in two lift types: hydraulic (pedal pump) and electric (battery-powered actuator). Each has a clear use case:

Hydraulic vs. electric lift comparison. For nursing homes, hydraulic is the more common choice — no charging, no battery replacement, works through outages.

Our view: for most nursing homes and assisted living facilities, hydraulic is the right default. It eliminates the failure mode that matters most in a care setting — the device being unavailable because someone forgot to charge it. If your facility has a high proportion of bariatric residents (over 130 kg / 286 lbs), an electric lift may be justified to reduce caregiver strain, but verify that the battery can complete a full day of transfers on a single charge.

Whichever type you choose, verify that the manufacturer provides load-test documentation. The relevant standard is ISO 10535:2021 — Hoists for the transfer of disabled persons[2], which specifies requirements and test methods for patient transfer equipment. A supplier who cannot reference or produce documentation aligned with this standard is a red flag.

3. Weight capacity and frame: ask about the metal, not the number

Weight capacity is the first number buyers look at. But the more useful question is what the frame is made of and how it's joined. A 130 kg rating on a mild steel frame and a 130 kg rating on a stainless steel frame are not the same product — the steel frame will show corrosion in shower use within 18 months; the stainless frame won't.

Three material questions to put to your supplier:

  • What is the frame material? Stainless steel outperforms mild steel in wet environments (shower transfers). Aluminum is lighter but typically has a lower weight ceiling.
  • How are the joints constructed? Welded joints are stronger than bolted joints, but they can't be disassembled for repair. Ask whether critical load points (seat post, caster mounts) are welded or bolted.
  • What is the base width? A wider base is more stable during transfers. A narrower base fits through standard doorways. The sweet spot for care facilities is 55-65 cm base width — stable enough for a pivot transfer, narrow enough for bathroom doorways.

4. Seat design: a transfer chair should do more than transfer

If the chair only transports a resident from A to B, you're getting half the value. A well-designed transfer chair replaces multiple pieces of equipment:

Every additional transfer is an additional fall risk. The CDC NIOSH Safe Patient Handling and Mobility program[3] identifies reducing the total number of transfers per resident per day as a core strategy for fall prevention. A chair that serves four functions means the resident stays in one seat through their morning routine — bed to chair, chair to toilet, chair to shower, chair to dining table — instead of being moved four times between four pieces of equipment.

5. What comes in the box — and what doesn't

The quoted price is only half the picture. Accessories that look included in the product photos may be line-item extras. Before placing a bulk order, confirm exactly what ships with each unit:

One more thing: order an evaluation unit before committing to a bulk order. One chair. Have your most experienced CNA use it for a full shift — bed transfers, toilet transfers, shower, dining. What is the difference between a transfer chair and a wheelchair?

A wheelchair is designed for mobility — moving a resident across distances. A transfer chair is designed for short, precise repositioning between surfaces: bed to chair, chair to toilet, chair to shower. Transfer chairs typically have a wider base for stability during pivot transfers, an open seat for toileting access, and often include a hydraulic lift for standing assist. They are not meant to replace wheelchairs for corridor travel.

How many transfer chairs does a 100-bed nursing home need?

A typical 100-bed facility starts with 2-3 transfer chairs: one for each wing or floor. Facilities with higher-acuity residents (more non-ambulatory residents) may need one per 30-40 beds. Ordering an evaluation unit first lets you measure actual usage before scaling.

Can one caregiver operate a transfer chair alone?

Yes. A transfer chair with hydraulic standing assist is designed for single-caregiver operation. The pedal pump does the lifting — the caregiver's role is guiding and stabilizing the resident. Coverage varies by jurisdiction and payer. In the US, transfer chairs may qualify as Durable Medical Equipment (DME) under Medicare Part B if prescribed by a physician for home use.
